Assessments

We complete assessments for re-housing, equipment, adaptations, a package of care, and advice about services available in the community, alongside advice.

We can advise towards equipment: Mobility equipment, Kitchen equipment, Equipment to support with transfers on and off bed, chair and toilet.

We can advise towards adaptations: Wet rooms, through lift, stair lift, Kitchen adaptations to safely be able to carry out kitchen activities.

Assessments and interventions can be carried out via video calls, Teams and Zoom. In person by the occupational therapist visiting your home, school, nursery or a safe place of your choice.
